MEDIA CONSENT AND RELEASE FORM

I/We, the parent (s)/guardian(s) of the above-named child do hereby grant ATHI LIONS FOOTBALL ACADEMY and their agents, the absolute right and permission to use photographic portraits, pictures, digital images or video tapes of my child, in which my child may be included in whole or part, or reproductions thereof in colour or otherwise, for any lawful purpose, by ATHI LIONS FOOTBALL ACADEMY, without payment or any other consideration.

I hereby release, discharge and agree to indemnify ATHI LIONS FOOTBALL ACADEMY from all claims, demands and causes of action that I or my child have or may have.
