I/We hereby grant consent to all health care providers to administer any necessary medical care as a result of injury/illness. This consent includes First Aid and Transportation to/from health care providers.

I/We also release ATHI LIONS FOOTBALL ACADEMY LTD from any costs/responsibilities arising from my son’s injury or illness whilst in training or participating in tournaments/friendlies associated with the Academy.
